The IRB is implementing electronic submission for the Exempt form. This data is shared by the investigator and the IRB Office and secured by the investigator's MNA account. Access the Exempt form on-line and follow the instructions under the Help button.

Exempt categories are included within the form.

In addition to the electronic submission, one hard copy of the form with hand written signatures is required to be submitted to the IRB office before the application can be reviewed.
