IMPORTANT NOTICE: THE ELECTRONIC FORM introduced in November 2007, must be used for all IBC applications. A single form may be used for recombinant DNA, microbe, and/or biotoxins registration. The form can be accessed by clicking on “IBC application” on the lower left of the ERMA home page at http://erma.musc.edu. After entering your user ID and password, you will be on the investigator’s home page. The menu on the left provides a link to instructions that also give access to additional references [e.g. NIH Guidelines for Research Involving Recombinant DNA Molecules (April 2002)]. Other links permit you to add a new form, view your existing forms, grant access to one or all of your forms to others, and determine the NetIds of your personnel. There is also a sample application (1) for reference.

PLEASE NOTE: Forms must be submitted in the principal investigator’s name and the PI must sign the assurance (section 7) before submission. If changes are made, the revised form must be resigned and resubmitted.

Eight MUSC IBC biosafety training modules (see Section 1e of the application) are available at http://www2.musc.edu/ibc/ibc.htm.  Quicktime (free software from Apple.com) is needed to view these modules.  Copies of the slides are available in PDF and Power Point formats from links on the home page.

A web-based e-mail system is available at the IBC site on ERMA so that an accessible log can be kept of the e-mails exchanged between each principal investigator and the IBC office. Please use it for all communications about your applications.

When registrations are approved, your approval may be printed out.

Terminations should continue to be submitted using the form (Word document) at the bottom of this web page.

Please contact Cynthia Karr at karrc@musc.edu or 792-3370 with any questions.
